Liability, Medical & Media Release

1. Assumption of Risk & Release of Liability

I understand that participation in dance instruction and related activities involves inherent risks, including but not limited to slips, falls, collisions, overexertion, equipment failure, and other injuries that may result in serious bodily injury, illness, disability, or property damage.

I voluntarily permit the student/participant to participate in all activities sponsored by Artistic Edge Dance Center, including classes, rehearsals, performances, competitions, camps, conventions, travel, and other on-site or off-site events.

On behalf of myself and the student/participant, I knowingly assume all risks associated with participation and observation of these activities and release, waive, indemnify, and hold harmless Artistic Edge Dance Center, its owners, directors, instructors, employees, volunteers, agents, and representatives from any and all claims, demands, actions, damages, liabilities, costs, or expenses arising from participation, except as prohibited by applicable law.

2. Personal Property

Artistic Edge Dance Center is not responsible for lost, stolen, or damaged personal property, including but not limited to clothing, dancewear, costumes, shoes, electronics, jewelry, or other personal belongings brought to the studio or any off-site event.

Students are encouraged to clearly label all personal items and avoid bringing valuables to the studio.

3. Student & Family Conduct

Artistic Edge Dance Center is committed to maintaining a positive, respectful, and safe environment for all dancers, families, staff, and guests.

Students and family members are expected to follow all studio policies and conduct themselves respectfully while attending classes, performances, competitions, events, or while communicating with staff through any medium, including email, text, social media, or online platforms.

Artistic Edge Dance Center reserves the right to remove or dismiss any student or family whose behavior is disruptive, abusive, threatening, harassing, or otherwise inconsistent with the studio's values. Removal from classes or events due to misconduct will not result in a refund of tuition or fees.

4. Medical Authorization & Health

I certify that the student/participant is physically able to participate in dance activities and understand that it is my responsibility to disclose any medical conditions, injuries, allergies, or limitations that may affect participation.

In the event of an illness, injury, or medical emergency, and if I cannot be reached, I authorize Artistic Edge Dance Center and its representatives to obtain emergency medical evaluation and treatment for the student/participant, including transportation to a medical facility if deemed necessary by emergency personnel or licensed medical professionals.

I understand that I am solely responsible for all medical expenses incurred.

For the health and safety of all students, I agree not to send my dancer to class when experiencing a fever, vomiting, diarrhea, or other contagious illness.

5. Photo, Video & Media Release

I grant Artistic Edge Dance Center the irrevocable, perpetual, worldwide, royalty-free right to photograph, record, livestream, and otherwise capture the likeness, image, voice, and performance of the student/participant during studio activities.

I authorize Artistic Edge Dance Center to use these photographs, videos, recordings, and other media for promotional, educational, advertising, informational, social media, website, print, or other lawful business purposes now known or later developed, without compensation or additional approval.

I understand that all photographs, videos, recordings, and related content remain the sole property of Artistic Edge Dance Center.

6. Acknowledgment

By registering a student with Artistic Edge Dance Center, I acknowledge that I have read, understand, and agree to these policies and releases and that they remain in effect for all classes, rehearsals, performances, competitions, camps, and studio-sponsored activities unless revoked or superseded in writing by Artistic Edge Dance Center.

Payment Policy Waiver Terms

By enrolling a student at Artistic Edge Dance Center ("AEDC"), I acknowledge that I have read, understand, and agree to the following tuition, payment, attendance, and refund policies.

TUITION

Tuition is assessed per dancer and is based on participation in the entire dance season, which runs from mid-August through mid-May, rather than the number of classes attended in any given month.

The annual tuition may be paid in one of the following ways:

  • Nine (9) equal monthly installments

  • Two (2) semester installments

Monthly tuition is due on the 1st day of each month.

September tuition is included with the August installment, and a final tuition payment is still due in May.

Tuition is not prorated for:

  • Scheduled studio holidays

  • Months containing fewer than four classes

  • Student absences

  • Missed classes

Likewise, no additional tuition is charged for months containing five classes.

Accounts with an outstanding balance after the 10th of the month are subject to a $15 late fee.

Any dancer whose account becomes more than one (1) month past due may be removed from classes until the account is brought current. Class placement cannot be guaranteed while an account remains delinquent.

ATTENDANCE & MAKEUP CLASSES

Students are encouraged to attend class regularly for consistent progress.

Makeup classes are available for absences due to illness or personal conflicts and must be scheduled in advance, subject to class availability and instructor approval.

Makeup classes are not provided for:

  • Scheduled studio holiday closures

  • Vacation breaks

  • Studio calendar closures

If AEDC closes due to inclement weather or an emergency, a makeup class will be scheduled whenever possible.

REFUNDS, CREDITS & WITHDRAWALS

All fees paid to Artistic Edge Dance Center are non-refundable, including but not limited to:

  • Registration fees

  • Tuition

  • Spring Performance participation fees

  • Costume fees

  • Performance ticket purchases

  • Camps, workshops, and special events

Credits will be issued only for the unused portion of prepaid semester or annual tuition when a dancer is unable to continue due to a documented injury. Any credit issued must be used within one (1) calendar year from the date of issuance.

No refunds or credits will be provided for:

  • Missed classes

  • Student absences

  • Scheduled studio closures or vacation breaks

  • Inclement weather closures

  • Voluntary withdrawal

A written notice of withdrawal is required at least one (1) month in advance. Tuition remains the responsibility of the account holder throughout the notice period.

PAYMENTS

Monthly statements and account information will be delivered electronically through the Parent Portal and the email address provided during registration.

Payments may be made by:

  • Parent Portal (Credit/Debit Card)

  • Cash

  • Check

  • Credit Card at the Front Desk

Credit and debit card transactions are subject to a 3% processing fee, which may change without notice. Processing fees may be avoided by paying with cash or check.

Families may also enroll in automatic payments through the Parent Portal.

PERFORMANCE & COSTUME FEES

Participation in the Spring Performance requires separate participation and costume fees, which are not included in tuition.

These fees are conveniently divided into three installments and billed during:

  • November

  • December

  • January

All performance-related fees are non-refundable once billed or paid.

ACKNOWLEDGMENT

By checking the acknowledgment box or signing below, I certify that I have read, understand, and agree to abide by the tuition, payment, attendance, withdrawal, and refund policies of Artistic Edge Dance Center.

I understand that enrollment represents a commitment for the full dance season and that timely payment of tuition and fees is required to maintain my dancer's enrollment.

Communication Policy

Communication Agreement

I understand that email is Artistic Edge Dance Center's primary method of communication and that it is my responsibility to regularly read all studio emails to stay informed about schedules, events, performances, tuition reminders, studio policies, and weather or emergency closures.

I agree to keep my contact information current in the Parent Portal and to add dance@artisticedgedance.com to my email contacts and safe sender list to help ensure important messages are received.

I understand that urgent or time-sensitive information may also be communicated via text message or through the Parent Portal, but email will remain the primary source of studio communication. Failure to receive or read studio communications does not exempt me from studio policies, deadlines, fees, or other requirements.
